- Enclosures
- Subjects
- Determination of Fire Origin and Cause
- Structure (or) Vehicle description
- Fire Scene Examination
- Investigation
- Comments
- Photograph Descriptions
- Diagram
You will have a verbal report of the cause of the fire within 24 hours of the scene examination. Your written report wil arrive by Priority Mail within 15 working days of the scene examination. Physical evidence from the fire scene will be secured when needed. The fire scene will be fully documented with photographs and/or video.

Assign a fire investigation to ISI with the following information:
- Who do we report to: Name, Address, Phone, Fax and Claim Number
- Name of the insured
- Date of Loss (and time if available)
- Loss location address or address of where vehicle is stored
- Name and telephone number of contact to gain permission to loss location
- Type of loss, Structure or Vehicle
- Commerical or Residential (Structure)
- Year, Make, Model, License and VIN (Vehicle)
